PROCEDURE FOR SPONSORSHIP OF A NEW ACTIVE MEMBER

[APPLICANT MUST FIRST ATTEND ONE AMED MEETING AS A NON-MEMBER IN ORDER TO BECOME ELIGIBLE FOR MEMBERSHIP.]

The sponsor must secure a new membership application:

The sponsor must give or send the prospective candidate the membership application for completion. All indicated sections must be typewritten and completed in full or the application will be returned without action. Do not send a C.V. in lieu of completing the application and do not put “see attached.” All non-applicable areas should be designated.

The prospective candidate must return the completed application to the sponsor who must verify that the application information is complete and typewritten. The sponsor then secures a seconding signature for the candidate. All applications must be received by the Secretary no later than July 31, prior to the meeting at which the candidate is to be considered for membership.

The primary sponsor must submit a letter to the Credentials Committee with a supporting rationale for his/her sponsorship of the candidate. The absence of such a letter will render the application incomplete.

The sponsor returns the completed and seconded application for appropriate action. All candidates must be interviewed by the Credentials Committee. Failure to appear will automatically defer the candidate’s consideration until next year, providing the candidate updates the information in the application and informs the Secretary that he/she wants to be reconsidered at the next meeting. A non-refundable $175.00 application fee must accompany the completed application, as well as the $300.00 guest fee for the meeting at which they are being considered for membership.

The sponsor or co-sponsor must be prepared to appear before the Credentials Committee if so requested. Sponsors need not appear unless summoned by the Credentials Committee. The sponsor or co-sponsor must have first hand knowledge of the candidate’s qualifications.

The candidate must be a member of the ADA or foreign equivalent. The candidate’s ADA number must be written on the application in the appropriate section.

PLEASE REMEMBER THAT PRIOR TO PROPOSAL FOR MEMBERSHIP, EACH CANDIDATE MUST HAVE ATTENDED AT LEAST ONE MEETING AS A GUEST!!! CHECK THE MEMBERSHIP CRITERIA CAREFULLY BEFORE PROPOSING MEMBERSHIP CANDIDATES TO MAKE SURE THEY ARE QUALIFIED FOR MEMBERSHIP UNDER THESE CRITERIA!!!
